Welcome to the Case Management Quality Self-Check and Review Tool!

This tool is part of Florida's Quality Case Management Toolkit. This toolkit was developed to create a shared understanding about case management between people who receive case management services, case managers, and the organizations that employ case managers. This tool is intended to support reflection and quality improvement and is not a test or formal performance evaluation. After you fill in the Case Management Quality Self-Check and Review Tool, you will see personalized results about case management performance. You can print your results, or if you want to provide your email address, we can email a copy of your results to you.

Who should use this tool and why?
Case managers and supervisors are encouraged to use this tool to reflect on service quality.

If you are a case manager, you can use this tool to:
  • Reflect on your performance and professional growth over the past 3 months.
  • Identify the skills you use most confidently and those you want to strengthen.
  • Prepare for supervision, coaching, or professional development discussions.
If you are a supervisor, you can use this tool to:
  • Reflect on how well the case manager you supervise demonstrates these skills.
  • Identify strengths and areas for growth.
  • Prepare for supervision, coaching, or performance discussions.
Do I have to use this tool?
No. Participation is voluntary, which means you are not required to use the tool. You may skip any questions you do not wish to answer.

Your employer or organization may ask or encourage you to complete the tool as part of professional reflection or quality improvement activities. However, your decision about how to respond—and whether to answer individual questions—is your own.

The questions should take about 10 minutes to answer.

What will happen to the information I put into the tool?
Responses will be collected and stored by Mathematica, a research organization hired by the Florida Developmental Disabilities Council (FDDC) to develop this tool and manage the data.

Your answers are saved as you enter them. The study team may include information from partially completed tools in the dataset, even if you do not finish all questions or click “submit.”

Mathematica will share deidentified and aggregated information with the FDDC. Identifiable individual responses will not be shared. The FDDC may use the deidentified data as part of statewide quality improvement efforts, which will help them to advocate for changes to improve case management in Florida.

What can I expect if I use this tool?
This tool is designed for self-reflection and quality improvement. Because participation is voluntary and you may skip any questions, there are no known risks to using the tool.

You are not required to provide personal information about yourself. If you choose to share your email address, it will be kept private and will not be linked to your individual responses in any reports shared with the FDDC or with the public.

If your employer or organization asks you to use this tool, they may decide how to use results as part of supervision or quality improvement activities. If you have questions about how results will be used, please ask your employer or organization directly. Mathematica will not share individual responses with employers.

How will you protect my information?
Mathematica will protect your information by storing all responses in secure systems. Only authorized members of the project team will have access to identifiable data.

Identifiable individual responses will not be shared with employers, the FDDC, or the public. Any information shared outside the project team will be de-identified or reported as a summary.

Yes, a fillable PDF version of this tool is available on the FDDC website.

You may choose this option if you prefer not to submit responses online. Responses completed using the PDF version will not be collected by Mathematica or FDDC and will not be included in statewide quality improvement efforts.
